Eureka, Ca., (KIEM)- St. Joseph Health offers medical respite care for homeless individuals in Humboldt County. The program helps patients once they are discharged from the hospital to recover in a safe, clinical environment. Services include resource referrals, transportation assistance, coaching, case management, and transportation assistance to follow-up doctors appointments.

The program has been so successful, the American Hospital Association (AHA) is taking notice. They’ve featured the transitional care programs as a case study in a special report. That report focuses on the role hospitals play in promoting healthier communities.
